HOW TO REGISTER

First please fill in the Personal Data Form and continue to registration by choosing the registration fee. During the registration process you will receive a password and automatically generated summary to your email address. Your password is the same for all on-line forms (registration and accommodation). Upon submitting the Registration Form, you will receive a confirmation from the Congress Secretariat within three working days.

For group registration (above 10 participants) please contact the Congress Secretariat at email address COS2019@guarant.cz .

^*)	Members of the Czech or Slovak Orthodontic Society - orthodontic specialists, full time in the Orthodontic University Department with at least 3 education posts for postgraduate orthodontic students (written affirmation).
^**)	Members of the Czech or Slovak Orthodontic Society – dentists in the postgraduate education for orthodontics, who are full-time in University Orthodontic Department (certified by the Head of Orthodontic Department)
^***)	Dental students (certificate from the University needed). The fee includes entrance to the congress for three days.

Registration fees include 20 % VAT. These prices are final. VAT will be applied under current legislation.

Note: Payments must be received by the corresponding deadlines (early or late), otherwise the subsequent rate will be automatically applied.

Methods of Payment

Registration fees must be paid in EUR by one of the following methods:

Please note that payments by bank transfer will only be accepted by September 12, 2019.

For payment by credit card please use the Online Payment System

Bank transfer

The bank details including the 10-digit variable symbol will be sent to you within three working days after the submission of the registration form. Please note that payments without this variable symbol will not be identified and the registration will not be confirmed.

Please note that payments by bank transfer will only be accepted by September 10, 2019.

After this date only payments by credit cards will be accepted via Online Payment System

All payments made by bank transfer have to be net of all bank charges. The payer pays the bank charges of their bank, and the beneficiary pays the charges of their bank, if any.

Cancellation and refunds

The Conference Secretariat must be notified in writing about the cancellation of registration by fax (+420) 284 001 448 or e-mail COS2019@guarant.cz . Telephone cancellations will not be accepted. The appropriate refunds will be made after the Congress. The following cancellation conditions apply:

From July 1 to August 14, 2019	50% registration fee
After September 14, 2019	No refund

If a change or cancellation made by a participant in registration or accommodation results in a refund, GUARANT International reserves the right to charge a 30 EUR handling fee.

Name change

If you are prevented from attending, you will be given the opportunity to send a colleague in your place. Name changes should be notified in writing to Congress Secretariat. A handling fee of 30 EUR will be charged for each name change. No name change will be possible after August 15, 2019.
